Is the best possible reward here. Anything else isn't worth the run from Qeynos to EK, and not worth having to make the run repeatedly until you do get it.

I recently did this, this is what I found. When talking to Plagus Ladeson the phrase: 'She's in East Karana.' did not work. You have to say 'She is in East Karana' to get the letter. When I gave Milea the letter, Faction hits as follows:
Steel Warriors has been adjusted by 5
Guards of Qeynos as been adjusted by 1
Corrupt Qeynos Guards has ben adjusted by -1
The Freeport Militia has ben adjusted by -1
Nights of Truth as ben adjusted by 1
Did this quest again to confirm if there was no reward. This time I got the Damask Cap. So it still gives rewards. Also got Golden Earring, Fur-lined Shoes, and Jasper as well as on the rare occasion, nothing but faction hits and exp.

Well I went to Freeport, got the note from Plagus Ladeson, walked to EC where I got a port from a friendly druid to NK.
I walked from NK to EK, all the way down the gorge, got the cloak, then tried to get back to Qeynos. I did this by going through HHP (Camo on) finding my way, eventually, to Kithicor, then on to Rivervale. Eventually found my way to Misty Thicket and a nice Halfling Rogue showed me to the PoK book there.
Total time taken: 1hour 45mins.
Total gain: +2AC +5HP
Like an idiot I stopped in Qeynos and asked Nerissa Clothspinner about her sister. She gave me a note to, yes, give to Milea Clothspinner for the "Clothspinner Sisters" quest. So now, my journey will be:
Qeynos -> PoK -> Misty Thicket -> Rivervale -> Kithicor -> Highhold Pass -> Eastern Karana -> Northern Karana -> Western Karana -> Qeynos Hills -> Qeynos
I'll pay Milea's rent if she'd just move somehwere closer to her family!!!

You should try Misty Thicket > Runnyeye > Gorge of King Xorbb > East Karana.

Last night I ran this quest for the first time. Here are the details:
8th Enchanter
Everywhere I went I was invisible. I stopped and prepapred to recast as soon as I began to fade.
Started: Warriors Guild, by the Arena, SW corner of FPWest. I just said "Milea is in East Karana" and immediately had the letter.
E Commonlands, West Commonlands: Ran through, no SoW present.
Kithicor: Fortunately, it was 8AM by the time I got there. Invisible, I ran through using a copy of the map from EQAtlas.
High Hold Pass: Entered, Invisbled up, Saw 1 Orc, and using a map (my first time through here) I worked my way through. NO other creatures seen. I think I got very lucky.
E Karana: Invisible, I ran down the long ledge. At the second Square tower (On the south wall; there is also a face carved into this wall) the slope down into the canyon was easily managed. I turned and ran (in the canyon) back towards HHP. About 1.5-2 minutes, I came across a tunnel under the overhead path I had run down and in that tunnel I found Milea. She is in armor and had black hair and a sword.
Her Loc was (-1808.17, -6069.36, 6.17)
I gave the letter and got a Rusty Spear. Rather poor item for all that work, but I got 2+ gold for it.
Gated back to the Soulbinder in FP. It was too late (in game terms ) to try and brave Kithicor so I camped out.
Time: Approx 40 minutes real time.

This is a much shorter quest as a 19 dru, use karana ring and gate to transit.
Milea gave out the following in order: Blackend Iron Coif (very nice), Lantern, Gold Ring, Backpack, Wood Shield, cloth Shawl, Griz Cape (nice). Good payout for 2 hours work total. Well , i have been , for probaly the last few weeks , focused on doing quests with mule charactors that give good rewards .So i was thinking of making a mule chanter in fp . getting to lvl 5 (gate and invis) and just doing this quest .
I would get note run to milea , get reward , gate back , repeat .

I too received a blackened iron coif from Milea, the fourth time I did the quest. The first three times I got rawhide leggings, a damask cap, and a lantern.

Did this quest on the way across the continent, I recieved Grizzly Bear Cloak and the other two in the party recieved junk.

First time I did this quest received the blackened iron crown...whoa so I did the quest 3 more times thinkin it was bugged...just got junk
