I spent a whole day farming the 66 second spawn that roams. Nada. I switched to killing both and it took a while but I eventually got the mob. He spawned on the one that doesnt roam. I feel pretty certain the non roaming 66 second spawn is the PH.
Oculys Overfiend is a very rare spawn I would guess <1% spawn rate.
I parked at +2700, +900 in the southern area of the NW bubble of Rathe to camp this guy. In this area you will find Cyclops, Bears, Lizards and Giant Skeletons. From what I noticed there is 1 specific PH on a 66 second timer (all other mobs are around 35 secs). The trick is to find the right PH and be able to hold that PH. Here's what I did. I Like many zones I ran my wizard around and grabbed all mobs and dropped a PBAE and started a count down. Once I found my 66 second PH I actually mezzed him on a bard and parked him off to the side for a bit. Once I had my PH parked off on the side I then proceeded to kill all the non PH's (Bears, Cyclops and Giant Skeletons) to convert them to lizards. My rational was simple. Bears are KOS to the mobs in the camp on the hill and would die randomly. Cyclops and Skeletons will agro you (and each other plus bears) and have to be dealt with. Having to deal with this constant cycle of spawns will make things far trickier for tracking. Once you chain kill the Cyclops, Skeletons and Bears after a few cycles they convert to lizards (repeat till all non PH mobs are lizards). The lizards are not KOS to anything, and will not agro at all unless you sit (assuming you are not low level). This allowed me to not have to worry about the trash mobs in the area chain cycling and messing things up. Once I had all trash converted to Lizards things became very easy. Just watch cycle.
Kill PH look for pop for Cyclops, Giant Skeleton, Bear OR Lizard. The first 3 made the PH very easy to spot as it's the only one in the area, the lizard you need to watch Track and make sure you are watching (I lost cycle a couple times and made for other trash popping up a few times). FYI trash in zone is constantly cycling all over so track can be a mess, so you have to watch closely.
I set up at the location listed above and killed the PH and waited. ALL PH's spawned a very short distance from that location (nearly all I could tag from that location, but a few spawned further south). In all it actually took me 199 spawns to get Oculys (that I counted).
Note later on I was testing things out and noticed there is a 66 second mob that paths OUT of this bubble of zone and paths around zone. I do not know if this is the PH for Oculys OR another named like Mortificator. You can set up near the exit of that bubble (Location +4000, +1790) and park and it will path to the exact same spot takes around 7-12 minutes between cycles to path to this spot. I did notice this roaming PH actually spawns in the same exact area as Oculys which leads me to assume it's his PH again. However I would not set up camp there as Oculys roams that same path and will be killed by one of the named guys in the tents on the hill and it would be far slower at best, or could be for Mortificator or another named. So if you don't find your 66 sec PH mob, chances are it's him and he's roaming zone and you need to do that ugly mess.
All the normal mobs around cyclops hill either respawn instantly or in approximately 30 seconds. Much like the named cycles in SKarana, Oculys PH spawned every 66 seconds. He doesn't appear to roam too far outside of this area, so should be fairly easy to isolate.

A bunch of the old world rares ph's all spawn at 66 seconds.
Took a long time to get this dude to pop, and during double weekend. I had a couple characters parked throughout the top NW hill area killing everything that moved. After about 12-16 hours of killing, my logs showed that he popped but was killed by the named Bandit in the tent. A few hours later he spawned again and I managed to get him before he pathed to the bandit.
So it looks like your mileage may vary, but killing everything in that hillside eventually got me the kill.
Maybe it was luck but followed the posted information killed PHs in his area as marked on Brewalls and described in below posts (the hill area near Lake Rathe zone in). Spawned in less than 30 minutes. He also dropped a Ruby Crown in addition to Totemic Helm.
Found him on track after clearing hundreds of mobs. He was already in a fight with Drayen Blackguard and losing, 7% down on Drayen and 20% on Oculys. I don't know if he always paths through there but he definitely was losing that fight and would have been gone if I was a minute slower.

Dropped him the other night fairly easy for my 46 bard just run around and twist the 3 DOT songs i have took at least 4-5 times longer then normal but worth it he droped the totemic helm and a blue diamond lol cant wait to see him again. P.S. ive kited Tarskus(Rng ivy etched quest piece)and was lucky i was a bard because he outran my sow so i had to strat my selos but this named cyclops was just as fast as a HG so he takes longer but he cant catch you if sown lol
Found him the other night and started to kite him and found he has a lot more HP than I anticipated.
I stacked Drones/Immolate/Winged Death on him and after two stacks he was not even down to half health. I did the trio again and before I could finish him I went LD. Good thing he was rooted at the time.
Came back and he was full HP and I was OOM (started at 3M) so I left him alone.
Kited this dude pretty easy...con blue at 51. Dropped Blue Diamond, Totemic Vambraces & Essence of Rathe. Never hit me but seemed to have around the same HP as Broog only was less resistant to my dots & roots.
killed this guy tonight was wandering by and he looked like he needed a beat down. to my amazement he had TONS of HP 3 Ebolts 2 plagues and alot of JBBs later I picked up a Blue diamond and Totemic greaves. I wasn't pre-paired for him, prolly best to malo him b/c high resist.
Most of it has stats that don't help much... for instance the gloves have +5INT which is useless to WIS casters. Totemic is good for Shamans up to about L30, by then you can acquire better stuff with WIS. The exception in the AC12 WIS+5 Helm, which I still wear at L50.

Killed him at 46(druid). hits like the others said up to 130(about). Was semi resistant to my firespells and semi to my magic based dot. Had to change tactics in the middle of battle as I was low on mana. just ran him up and down the wall reapplying my dot... lots of hitpoints( my guess is around 6k hp. I didn't notice an hp regen though. As I said before I had lil to no mana, so I couldn't reapply my dot without interuptions. He dropped totemic bracers and a ruby crown, also some plat.
I was able to kite this guy (50 druid) was a blu con... when i went lom a kind warrior ran him around so i could med for another blast/dot.. he droped the helm and a blu sapphire that night.
I was on my way out of Rathe Mountains, headed to Lake Rathe, and this guy is on track. Long kite, but he goes down and drops Totemic Cloak, ac10 sta4 or something like that, shaman only.
I have bad luck with rare spawns and this is the first rare spawn I've seen in rathe mtns, with the exception of having Zephyl show up once to test my fear song out while I was kiting a hill giant. Even seeing this guy was just some other group taking him on so no loot for me but here's what I noticed. Blue at 51, hit into the 130s, and the ton of hitpoints thing seems wrong, it appeared that he had it all in regen as the group fighting him could barely keep his HP bar from going back up. Anyhow, I managed to help them out by putting fear and chains on him (that's fear kiting for all the non bards out there) and eventually had to help melee but between me and their group concentrating on offensive power he eventually died. They said he dropped totemic boots, he gave no faction hits.
Killed this rare spawn last night on Povar. The Group was me ( 42 wizard), my Guild master (48ish wizard) and a 45 cleric. I won the roll on the BP, but we also looted a gem (ruby) and some cash. This MOB had an insane amount of HP. The 48ish wizzy did over 3k and I did around 2k, and the cleric even dropped a few bombs in. Rough MOB, but it was a fun group. Thanks Guys.

Just smacked this guy in RM, he was like a regular clops as far as spells, no resists. Myself 53 ranger and a 40 ranger kited him up and down the wall by LR zone a few times, lots of HP and hits very hard. Loot was a totemic bracer and a blue diamond, plus some paltry cash.
