I never get a vial of vigor when I give her the shroom...am I doing something wrong or did they change it?

Give her a mushroom, then drink the potion, then give here another mushroom and carry the potion till the old one runs out..

Give her one of those blue mushrooms found behind Rytan(buffs and spells). Then you will get a shimmering vial of vigor.
