There are two quest versions of this NPC for low-level characters to get guild faction in Neriak.
The Grobb version (named Savarixsa Zexus) is confirmed as giving the Note of Recommendation and working.
This East Freeport version gives the note, assumed the quest still works (as we believe it is the same quest with just a different start).

She is not attackable. No spells, taunt, or attack. In fact she will do nothing to you even if you are a lvl 10 KOS standing next to her. I know =)
She is still level 40. She only responds to hails starting at apprehensively I believe. However, if you say I am "devoted to innoruuk" targeting her, she will hand you the note regardless. All the NPCs in the turn in room also suffer are non-combatants. Screenshot uploaded. FYI, there is a second entry for this NPC at lvl 50

As an Erudite Necro on TLP server I got the letter from her using the text as per Sylphan's old post. I conned Dubious to her and she did the usual insult text at me from hails.

(You need somewhere between -274 and -250 faction with Priests of Innoruuk). I know it works at -250 and is rejected at -275.
She is no longer attackable apparently... Went to kick her butt for the faction but nothing happened...

Usually see a few corpses of good-aligned players around where she stands in the giant pit in the revamped Freeport East. Caught me by surprise once on my lv 2 monk, as I had no idea I could get killed in my own hometown.

By her faction adjustments for kill, I would guess that she is on the Priests of Innoruk faction. I con dubious to her, but warmly to Dismal Rage and indifferent to Opal Darkbriar herself.

The answer to all of this is the following:
In Grobb at the shammy guild (-53, -433) there is 'Savarixsa Zexus' which give you the note to raise your Innoruk faction . Well the note is sigend by Zaxun but well - it works.
In Freeport East at the dark side Cleric GMs there is 'Savarixsa Zaxun'. I didn't manage to hail or even speak to her. (also tried to hail her as Zexus).
I reported this bug, let's see if they fix it.

The note of recommendation she gives you is aimed at human clerics who worship Innoruuk.
When you bring your human cleric to the Neriak Third Gate cleric guild, the cleric guildmasters there will initially con dubious and if hailed will say "You could never be of service to our guild."
However, once you hand in this note of recommendation, your faction with Priests of Innoruuk goes up to indifferent, and you are given a quest to go kill a halfling raider and return one of their leather skullcaps.
Anyhow, this is a useful faction correction if you wish to do the regent symbol of innoruuk and related quests with a human cleric worshipping Innoruuk.
Your faction with the cleric guilds in North Freeport, Qeynos, and Felwithe does go down with this faction adjustment, but is still in the dubious range where you started. As you do more quests with priests of Innoruuk, however, your faction with these other cleric guilds will go to kill on sight if you have not done other quests to improve those factions in the meanwhile.
The only faction that is a particular challenge to raise is the Priests of Marr in North Freeport. Basically, the only reasonable way for a dubious char to raise their faction is to kill Freeport Militia, which lowers faction with your own guild - Dismal Rage as well as with the Freeport Militia. But I would advise not to do that unless you have already raised Dismal Rage faction and Freeport faction in other ways and have room to take a bit of a hit, and even then, be cautious.
Dismal Rage faction can be raised with bone chips, spiderling silk, and cyclops eyes to Xelha. Militia faction can be raised with the two rogue quests involving bringing messages to Janam and Rebby and returning with the response from Janam and whisker from Rebby.

This also works for Dark Elf necromancers, shadowknights, and clerics - even some troll shamans though getting to the dismal rage guilds is hard if you can't fight the drowned citizens in WFP.
Anyway, at level 7, each raiders cap yields about 3.5 blue of experience, very fast leveling considering you can usualy purchase them for a plat of vendors in Nek Forest and Neriak FOreign Quarter.

Iksar and troll shammys can now use the Cazic-Thule symbols from Paineel (with a lot of faction work), so I would think they've altered the Innoruuk symbol so Human clerics can use it, too. Even if they can't wear it, I'd do the quest anyway for roleplay reasons.

she have me "note of recommendation" and told me to take it to her brother in Neriak.
Actually, she said to deliver it to her brother, High Priest Perrir at the Spires of Innoruk.
