According to the patch message of 5 July, 2007 the Hatchet and Redfang events in Demi-Plane of Blood Tier 1 will now drop this back-flag item.

Yes, it works as a substitute for completing the five pre-Demiplane raids (Shyra, Bloodeye, Draygun, Council of Nine, Sendaii). You'd still need to get a Monocle of Blood first though.
