Bregna may be found at -62, -456, 74. At the end of the previous quest, Kaglari asked you whether you wanted to "more help Innoruuk.".
You say, 'I will help' Bregna says 'Me hears orcs nearby are trubble. Da werd frum Neriak is dey wants us ta kills dem before dey organize. Dark elf say ta looks for Deathfist Clan. Say dey called cen-tu-ri-ons. Dey try ta gets big orc army. Shows me ya can strike fear and hate inta dem orcs.. dey needs be more scared a us den dem humies. Brings me a Deathfist slashed belt.' Give Bregna one Deathfist Slashed Belt. Bregna says 'Good job. Dat help lerns um. Takes dis ta help ya lerns how ta do more hateful tings. Ya gots a good starts fer Him ta be prouds a ya.' Brenga would give me the initial quest dialogue (new dialogue) , but he would not respond when I clicked on 'help'. He also did not respond to 'Meez more help Innoruuk'. He did respond to 'I will help', which did give me the dialogue about the orcs being trouble.

Unresponsive to all my ideas to trigger 2nd quest. Any ideas?

Bregna at /loc -62, -456, 74 gave me all the quest text, faction hits and spirit pouch when I handed her a deathfist slashed belt.
