You sure it wasn't spectral harbringer ? The Kunark-era clicky armor pieces tended to be very specific drops, and it would be surprising for a new drop NPC at this point unless something has been purposely changed?

I'm positive. We were outside of the second crypt where Chottal spawns and the spectral commander was the only one of the two gaurdians up that flanks the crypt door. We killed him and he dropped this. The whole group was shocked.
This occurred on Lockjaw server.
Looking back through the logs we looted this at 6:30pm and the two nearest spectral harbinger kills were at 6:01pm and 6:32 pm. spectral commander killed at 6:28pm.
