In the Plane of Storms: Spawns during the encounter with Jeplak, Lord of Srerendi when someone in the raid group dies.

In Vergalid Mines: Vergalid's End: Part of the encounter with Goru Uldrock, the Reincarnate.

In The Well: No Freedom in Death: Spawns as part of the task "No Freedom in Death".

In the Convorteum: Part of "The Stone Warden" event (see this quest entry for event information and loot).

In Sepulcher West: Part of the raid event "Two Faces, One Vision" (see this quest entry for event information).

In Chapterhouse of the Fallen: Upper case "A Lost Soul" is a named known to spawn in the basement at location 450, 245, -222 (PH "an enraged willow wisp"). "Flame Spiral" is cast every 20 seconds on random target (but not most hated). "Aegis of the Fallen" (spell rune) is cast every 40 seconds. The lower case "a lost soul" is a common trash mob in the prison area.

These are spawned (along with these guys: Mangled Travellers) whenever someone (or some thing) on your side dies in the fight agaist the REAL Jeplak, Lord of Srerendi. I say "some thing" because this includes PETS (you might want to hold the leash on all those AA pets during this fight, hehe). This restriction applies even after the named is down - you have to kill all these adds without dying to have a clean fight.

By themselves, these aren't so tough. They hit for a max around 300, they don't have any more HP than the stuff by the zone-in, and they are straight melee. Slowable, nukeable, rootable/snareable, they're only hard because there can be LOTS of them, and they swarm to whomever has the lowest health like sharks. Kite them, pick them off one by one (or just AoE bomb them, hehe), and they're raid exp. All of this info is correct

My guild just attempted to kill Japlak and I just wanted to add that not only do they spawn when someone (or something) dies, they also spawn automatically after engaging Japlak. Our raid wiped rather quickly when 8 of these poped up out of no where even though nobody had died yet. The adds are mezable however.
