This can be given as in the quest "Plaguebringer Proof", but it also can be handed back to Sragg.

Sragg Blood says, 'What have you brought me, worm? Hmm... This is disappointing. That Tovax has failed us for the last time. Your loyalty to Bertoxxulous and the Bloodsabers has been noted. Now, if you will excuse me a moment...'

Your faction standing with Bloodsabers got better.
Your faction standing with Guards of Qeynos got worse.
Your faction standing with Opal Darkbriar got better.
Your faction standing with Priests of Life got worse.
Your faction standing with Corrupt Qeynos Guards got better.

at this point, Tovax spawns in front of Sragg, begins buffing himself and summons a pet. A minute or two passes until they strike up a dialogue:

Tovax Vmar says, 'Huh? What the... oh, no... S'ragg! PLEASE NO! Bertoxxulous save me!'
Sragg Bloodheart says, 'I will hear no more excuses, Tovax! Enjoy eternal pain and suffering!'

nothing happens after this. You can decide to attack Tovax, indifferent and has no faction hits. Quest is obviously broken at this point (thanks, Sony). Drops random junk loots.

Tovax Vmar says, 'Ooh... Aaah! No! Please! Ouch... Aarghhhhh!'
