Who or what drops this? Is it infact triggered or worn? Some information would be great, Thanks. I dont hit this site very often so if you could email me what information you have it would really help. Thanks again
Actually this effect, despite Allakhazam's listing, is in fact worn. Also, in regards to where you get it, mine dropped off of a Siren in Siren's Grotto. I hope this helps.

I've got one of these...I was fortunate enough to find it at the GD rings on the ground...no one ever claimed it. Ive used it for sime time now in various situations and after going to the Grey the other night and talking with friends about it...I noticed that the word (worn) was now written after the Enduring Breath effect....this wasn't there before. I just makes me wonder if there is a certain time limit on usage for it.
(worn) means that the effect is permanent. There's also (combat) which means that it's randomly triggered during combat and one other for triggered items (don't remember the text on that one).
This was added recently to make some of the items easier to understand for some players.
So don't worry, your item won't wear out any time soon!
(Worn) means that you have to equip the item in order to use the effect and it's permanent. This item happens to have a permanent effect of EB, but this is not always the case. Triggered from inventory reads (Casting Time x sec) and Triggered while wearing says (Must equip. Casting Time x sec).
I prefer triggered honestly when it comes to EB items...I can cast it on other people who aren't as fortunate to have obtained an EB item...
As for the rest of that post (Combat) does indeed mean that it's a random proc.
This is incorrect. Items with spells come in four types:
1. Proc, which will say (Combat) 2. Permanant, which will say (Worn) 3. Right clickable without wearing, which says (Casting time XXX) 4. Right clickable only while wearing, which says (Must equip, Casting time XXX)
I can tell you've never been on a Kedge Keep raid. On raids to Kedge it's quite typical to have more people die of drowning than to enemy action! This was indeed the case on the one I was on... ironically one of the deaths was a mage who had just given me 8 waterstones he summoned (should have kept one ofr himself). It's very easy to catch yourself out of mana when your EB is fading, have EB dispelled by a mob down there (several of the casting mobs love to dispell your buffs, and that includes EB!) and there's the simple case of being distracted in battle and forgetting to check wheter EB is still up. As a person in there who was depending on waterstones, I always kept one in a hotbutton and kept a close eye on the EB icon... plus was grouped with a bard who periodically sang Tarew's Aquatic Ayre as a backup, just in case (do not have a bard as your ONLY source of EB! But it's a good backup.)
Go on a single Kedge raid, and you'll quickly see the value of any item that has a permanent EB effect on it. The same holds for The Grey.
Not sure if the price on this is still right, because I have yet to see a single permanent EB item in the bazaar for less than 2k... whcih is about what the rebreather costs, and this has the clear advantange of being much lighter (something that monks or people with low str will appreciate!). Also melees are more likely to have better stats on their head item than their neck item, so would rather be wearing tihs than the rebreather. Myself, I'd expect to pay at least 3k for this... though maybe I can find a bargain on this in the bazaar someday!
Ever see someone forget to rebuff EB? Or Go to KK go OOM and can't rebuff? Now can you see why it's worth so much? Why do I ask so many dumb questions?

Sure it does. This way the effect can be dispelled by the mob you are fighting. It makes perfect sense. . . Is it better than a fishbone earring? I sure dont think so, but hey, I have one.

I believe you are mistaken the "Runed Shell Necklace" has a charge of 5 but the "Ornately Runed Shell Necklace" could very likely have unlimited charges triggered or be a permanent effect. From what I've heard was that the triggered ones need fishscales to work on top of the charges.
It's permanent, no need for right click it. Just equip it in the neck slot and go to town. I slap it on my rogue, go sneak/hide, jump in the water in NFP and go to bed. Wake up next morning and my rogue is a master swimmer :)
