It means you can take this item and combine in with one of your items.
Basically you look at the restrictions info on the stone, and then combine this stone with a piece of your equipment that falls into a category within the restrictions.
You combine the items at a wayfarer camp's bird feeder. The stone disappears and the item is returned to you with the stat associated with the stone.
Its important to note the item becomes no drop so the best thing to do is to augment your no drop items first.

We found this on about the 4th mob in a takish level. Was +5 tho, so im guessing it probably has varying stats depending on what level your dungeon is.

I obtained this item in Rujarkian Hills off of one of the random orcs. Group encounter, Normal Risk. All players were level 65.
