Just had this drop from Captain Nathan Flock as well in Tower of Rot (08.03.14)
While the shroud is equipped/evolving, the player experiences various curse effects. Although the effects seem to trigger randomly, it does seem that the effect is based on the skills utilized by the player. Essentially, the item randomly cripples the abilities you are using. Searching Lucy by 'terrible curse" returns the following list:
Curse of 1H Blunt
Curse of 1H Pierce
Curse of 1H Slash
Curse of 2H Blunt
Curse of 2H Pierce
Curse of 2H Slash
Curse of Abjuration
Curse of Abjuration
Curse of Alteration
Curse of Alteration
Curse of Archery
Curse of Backstab
Curse of Block
Curse of Conjuration
Curse of Conjuration
Curse of Defense
Curse of Dodge
Curse of Double Attack
Curse of Dual Wield
Curse of Evocation
Curse of Evocation
Curse of Flying Kick
Curse of Frenzy
Curse of Hand To Hand

I can confirm that, as a warrior, I have been cursed by most every warrior-applicable effect on that list at one point or another.
Also, if you unequip the shroud, you get nailed with the effect Torn Soul -- which caps your h/m/e at 25%. Definitely don't want to unequip while in combat!

snares my beast periodically as well. Really annoying when roaming around a wide area and fighting

Lol, I did just that and my group was like, "Dude. What's wrong with you. Is your health not updating. Is the cleric merc not healing you?" I had no idea what the Shroud of the Forgotten was, hadn't looked it up and it wasn't on the Magma Behemoth loot tables at least at that time, which is where I got mine. So I just put in on and pulled it off, not immediately noticing the fun Torn Soul effect. Now to kill 6,000 green mobs to evolve it, which is the rumor I hear.

A back item that eventually evolves a shoulder item? Whats next.... an extension (CotF) that evolves into an expansion?
